For patients interested in replacing existing amalgam fillings, how those restorations are removed can be just as important as the materials chosen to replace them. One approach used by some biological and holistic dentists is the SMART protocol, which stands for Safe Mercury Amalgam Removal Technique.
Developed by the International Academy of Oral Medicine and Toxicology (IAOMT), SMART outlines specific safety measures intended to reduce mercury exposure during amalgam removal.
Why Does Amalgam Removal Require Special Precautions?
Dental amalgam contains a combination of metals, including elemental mercury. Drilling into an amalgam restoration during removal can generate mercury vapor and particles.
SMART incorporates protective measures designed to minimize unnecessary exposure for the patient, dental team, and surrounding environment during the procedure.
What Happens During SMART Amalgam Removal?
A SMART appointment involves multiple layers of protection. Depending on the clinical situation, these can include a dental dam or other isolation technique, high-volume suction positioned near the tooth, additional air filtration, protective coverings, and an external source of air or oxygen for the patient.
Rather than simply grinding away the entire restoration, the dentist may section the amalgam into larger pieces whenever possible. This technique can reduce the amount of drilling required.
What Happens After the Filling Is Removed?
Once the existing restoration has been removed, the tooth is thoroughly evaluated. Any necessary restorative treatment can then be completed using the material selected for that patient’s needs.
Not every tooth or restoration is the same, so treatment recommendations should always be based on an individual examination.
An Informed Approach to Amalgam Removal
At Assure A Smile, we believe patients should understand both the materials being used in their dental care and the procedures involved in replacing existing restorations.
